The 3 Options is brief on below :

Always Allow : If you select this option this means you have permitted your device to download any size of apps with Mobile Data or Cellular Data for your iPhone & iPad.

Ask if Over 200 MB : if you select this option this means you have permitted your device to ask if the apps over 200 MB and connected with mobile data . So if any apps is over 200 MB it will ask you a Permission with a Popup for proceed download or cancel download . you can choose whatever you want . 

Ask First : if you have selected “Ask First” it will always ask your permission for download. It doesn’t matter the apps size. It will always ask you to download now or later with wifi if you are connected with mobile data or cellular data.
